MEDICAID 

SERVICE COORDINATION

MEDICAID SERVICE COORDINATION (MSC) is a Medicaid State Plan service provided by OMRDD which assists persons with developmental disabilities and mental retardation in gaining access to necessary services and supports appropriate to the needs of the individual. MSC is provided by qualified service coordinators and uses a person centered planning process in developing implementing and maintaining an individualized service plan (ISP). MSC promotes the concepts of choice, individualized services and supports and consumer satisfaction.

JRC offers a variety of residential opportunities for individuals with developmental disabilities in Jefferson County through one of our 29 Individual Residential Alternatives or 3 Intermediate Care Facilities. These sites allow a person and their families to select a site based on individual needs, abilities and preferences. The certified capacity of the residential program is 216. We also have sites that are available to provide respite services both hourly and overnight for families who need a break or have plans that require their son or daughter to be supervised.

The residences range in size from 2 to 13 beds and are distributed throughout Jefferson County. The 3 Supportive IRAs provide up to 21 hours of support a Chili Cookoff week while the Supervised IRAs, Community Residence and ICFs provide 24 hour coverage and are staffed based on the supervision and assistance required by the residents. There are both co-ed and single sex sites and they all provide a homelike atmosphere that provides training in activities of daily living, self-care skills, community inclusion, socialization, and many other skills to develop independence and personal growth.

The community residence is a large group home that provides services to thirteen individuals in varied activities as described above. The 26 Individual Residential Alternatives provide similar programming when compared to the community residence; however they are generally smaller in size and provide the opportunity for more individualized attention. The 3 Intermediate Care Facilities are 12 bed sites that serve those with a higher level of need such as the adult medically frail, geriatric and young adult/children. The 3 Supportive IRA sites provide support and guidance to adults with developmental disabilities who require minimal supports to be successful living in the community and average 12-15 hours of staff support a week. We have a respite bed at the Chaumont Community residence, Pearl Street IRA, Adams Center IRA, Adams IRA and two beds at the ICF in Dexter, NY.

In addition to these programs we provide residential habilitation programs in the homes of individuals in the community. Training is individualized and based on the needs and preferences of the individual. Residential Habilitation is provided after approval from Sunmount DDSO and the availability ofAdopt a Platoon photo funds. We also provide family support services. These services provide respite services for families for their child or relative, training in the home or outside environment to assist in maintaining their ability to stay in the home and in emergencies help provide equipment/supplies to assist the person in their activities of daily living.

Staff qualifications to work in the residential program include but are not limited to:

  1. High School Diploma or GED
  2. Acceptable Driving Record
  3. Acceptable Background Check
  4. Training will minimally include – emergency procedures, fire safety, community first aide and CPR, 633 and 624 regulation training, Strategies for Crisis Intervention and Prevention, characteristics of human development and other pertinent classes to assist in the provision of quality services to individuals with developmental disabilities.

The residential program completes satisfaction surveys with the residents as well as a sampling of family members to insure programming is based on the needs, desires and choices of the resident. We also have a residential advisory board made up of residents from the sites to discuss systemic issues, address any concerns and make recommendations. In addition to this many of our residents belong to the North Country Self-Advocates, which is involved in the advocacy movement on the local and state level. The Interdisciplinary Treatment Team reacts to the programmatic needs of the residents and management works with the individual, family, advocates and staff to address other issues that are identified.

To be eligible for services in the residential programs the individual must have a developmental disability that occurred before the age of 22 and has demonstrated adaptive deficits per the guidelines of the Office of Mental Retardation and Developmental Disabilities.
